New Patient Registration
When you register you are asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.
There are links to the registration forms you require, below. Please print them, fill them out as soon as possible before you require an appointment with a GP or Nurse, and bring them into the Surgery, (ideally a week to 10 days before). In addition please bring proof of address and identity, for example passport and a recent utility bill.
If you do not know your NHS number please contact your previous surgery and they will give it to you. Please complete the forms fully to avoid any unnecessary delays with your registration.

New Patient Registration Forms are also available from the reception.
You can request an appointment with any of the clinicians who have an appointment available at the time you wish to attend.
A visitor to your home may be treated, if necessary, as a temporary resident, once they have completed the appropriate form. If, however, they intend staying more than three months, it will be necessary to fully register.
This is confidential, and is used to help us to give you the best possible care, as we will not have your medical records immediately.
